*Overview*
We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a crucial role in coordinating care and support for clients, ensuring they receive the necessary resources and services to improve their overall well-being. This position requires strong interpersonal skills, the ability to work collaboratively with various stakeholders, and a commitment to advocating for clients' needs. This position is Sunday-Thursday 0800-1600.
*Duties*
* Data input into HIFIS software system
* Work from a Housing First and Trauma Informed focus
* Coordinate services and resources by collaborating with healthcare providers, social services, and community organizations.
* Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes.
* Maintain accurate documentation of client interactions, services provided, and progress reports.
* Provide support and guidance to clients and their families regarding available resources and options.
* Facilitate communication between clients and service providers to enhance understanding and cooperation.
* Coordinate with support staff and/ or placement students when applicable, ensuring adherence to best practices in case management.
* General house maintenance - grocery shopping, room cleans, laundry, sorting donations and light general cleaning of household chores

*Experience*
* A background in social service work, or a related field is preferred.
* Previous experience in case management or a similar role is highly desirable.
* Strong organizational skills with the ability to manage multiple cases simultaneously.
* Excellent communication skills, both verbal and written, for effective interaction with clients and professionals..
* Proficiency in using case management software and other relevant technology is beneficial.

Pay: $22.05 per hour
