*Position Title:* Nurse Practitioner
*Reports To:* Director, Clinical Services
*Position Summary:* The Nurse Practitioner, Primary Care (NP) will work within a multidisciplinary care team to provide comprehensive holistic patient centered primary care to patients of all ages. The NP will panel patients and support access and attachment strategies as an employee of the Calgary Rural Primary Care Network (CRPCN) and will work out of CRPCN Community Clinic sites. As a primary care provider, the NP will work with a high degree of professional autonomy and judgment. They will offer services based on scope of practice, training, and expertise.
*Key Responsibilities: *
· Primary care service delivery for all ages, including but not limited to chronic disease management, family health, early stages prenatal care, and seniors health.
· Conduct advanced, comprehensive and/or focused health assessments
· Diagnose medical conditions and disease and ensure screenings are complete
· Order, interpret, and perform diagnostic tests Prescribe pharmaceuticals, medical devices, and therapies
· Make referrals to specialists
· Perform non-invasive and invasive interventions if within scope of practice (ex; PAP tests)
· Provide care as Most Responsible Provider
· Use a patient-focused care model that involves patients, families, and caregivers
· Act as a mentor and coach for colleagues
· Coordinate and collaborate with other healthcare providers including allied health, other primary care providers, agencies, and community resources to provide comprehensive patient/family centered care
· Participate in CRPCN and ongoing quality improvement initiatives
· Participate in CRPCN evaluations as required
· Other duties as required within Nurse Practitioner scope of practice and in alignment with CRPCN access and attachment strategies

*Qualifications:*
_*Education/Certification:*_
· Master?s degree in nursing, with Nurse Practitioner certification
· Fully licensed and in good standing with regulatory body
· Current BCLS certification for health care providers
_*Experience:*_
· 5 or more years of nursing experience, preferably in a rural primary care setting
· Experience working within a multidisciplinary care team model
· Knowledge of chronic disease management principles and processes using current best practice guidelines
· Knowledge of EMRs and clinical process redesign is an asset
· Knowledge of and interest in principles of a population-needs based approach, primary care, primary health care and the medical home model is an asset

· Afternoon or weekend work may be required
· Travel to additional CRPCN work sites may be required
· Valid driver?s license & access to reliable vehicle required

The Calgary Rural PCN works together with communities, family doctors and other health team members to do whatever we can help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through supports we have in our member clinics, such as nurses, dietitians and other health care professionals. We collaborate to provide the tools and education to empower patients to work on their everyday health needs.
