We are looking for a full-time, permanent Nurse Practitioner to join our growing multi-disciplinary health team and to provide primary care in a new innovative Seniors Clinic in Kitchener, ON. Our mission is to help people live well throughout their experience of later life by providing care, education, training, and service.
Plum Tree Memory Care?s main focus is complete, compassionate care to individuals and families living with memory concerns. Our experienced team provides personalized memory care services and education to help people live well?whether at home, in a retirement home, or in long-term care. Plum Tree Memory Care was created to complement the foundational work of MINT Memory Clinic, a non-profit organization pioneered by Dr. Linda Lee to enable better dementia care in primary care. Plum Tree Memory Care is built on MINT Memory Clinic?s standardized, nationally-accredited training program, ensuring services are informed by evidence and experience.
The new Seniors Clinic is a collaboration between Plum Tree Memory Care and the Centre for Family Medicine Family Health Team (CFFM). CFFM is an academic Family Health Team comprised of six sites and is an integral part of the University of Waterloo Health Science Campus as well as the McMaster Waterloo Regional Medical Campus.
We work in a fully computerized environment and encourage continued educational experiences.
Plum Tree and CFFM have innovated many new primary care clinics. Within this Seniors Clinic, the nurse practitioner will provide direct acute and chronic care for patients age 65+, with a strong emphasis on health promotion, disease prevention, and dignified end-of -life care.
The Seniors Clinic will have an interdisciplinary team of health care professionals who work collaboratively to provide excellent care. The nurse practitioner and patients of the clinic will be supported by family physicians, nurses, pharmacists, occupational therapists, receptionists and other health professionals.

*Duties*
- Provide primary care to patients age 65 and over
-Conduct history and physical examinations
- Diagnose and treat common acute illnesses and injuries
- Manage chronic health conditions
- Order and interpret appropriate diagnostic tests
- Develop and implement management plans
- Educate patients on health maintenance and disease prevention
- Collaborate with physicians and other health professionals
*Requirements*
* Master?s preparation and current CNO Certificate as RN (EC)
* Minimum two years of experience working as a nurse practitioner in primary care including chronic disease management and episodic care
* Demonstrated ability to work independently within the NP-PHC scope of practice
* Demonstrated ability to work collaboratively with family physicians as well as other health professionals
* Demonstrated sound clinical judgment skills, excellent health assessment skills, diagnostic and therapeutic skills
* Interest and/or experience in care of the elderly
* Willingness to learn about cognitive impairment, end-of-life care and other topics relevant to caring for the elderly
* Awareness and understanding of equity, diversity and inclusion
* Vulnerable Sector Check required
