The Alzheimer Society of Brant, Haldimand Norfolk, and Hamilton Halton (ASBHNHH) is looking for a passionate and qualified Counsellor to provide counselling and support services to persons living with dementia and their care partners, in accordance with the Alzheimer Society?s Strategic Plan. These services are delivered in-office, by home visits, and by phone, using both individual and group approaches.

This position is based in our Hamilton office with travel throughout the greater Hamilton area. Office hours are Monday to Friday from 8:30 am to 4:30 pm, but occasional evening or weekend work may be required. The salary range for this role is $48,867 to $61,079 based on 1820 hours annually (35 hours per week).
*Duties and Responsibilities:*
* Provides supportive counselling and one-to-one education to persons living with dementia and their care partners
* Facilitates support groups
* Participates in care review meetings with ASBHNHH colleagues as well as external professionals
* Enters, updates and maintains client notes in the organization?s database
* Identify support service needs for persons living with dementia and care partners
* Liaise with and link clients to AS education and support services, and to appropriate community agencies and resources based on their needs and requests
* Develop a plan with persons living with dementia and their care partners which includes referral to services as needed, such as assessments, respite options, Safely Home program, financial planning, home support, placement in long-term care etc.
*General Competencies:*
* Must have knowledge of client safety issues and a demonstrated capacity to apply client safety practices.
* Must have knowledge of consent and capacity issues and legislation related to high-risk elderly.
* Must have knowledge of community resources.
* Excellent verbal and written communication skills
* Ability to work independently in a professional, self-directed, and collaborative manner.
* Proficiency with Microsoft Office Suite applications and databases
* Knowledge of dementia and other age-related conditions
* Must have a valid driver?s license and access to a reliable vehicle
* Must provide a clear vulnerable sector police check
* Fluency in a second language is an asset
*Education and Experience Requirements:*
* University or College Degree in Social Work (MSW, BSW, SSW) or comparable university degree or pedagogical preparation
* Minimum of two (2) years of demonstrated experience working with persons living with dementia and care partners
* Member of a professional college
* Support group leadership experience
