The Pediatrics on Princess (POP) Centre treats children with social-emotional, behavioural, and academic referrals who also encounter barriers related to adverse childhood experiences (ACEs), poverty, and trauma. The Community Social Pediatrics (CSP) Advocate is part of the POP Centre's Clinical Team and the primary contact for most families. The Advocate may have a background in Social Work, Community Service Navigation, or another similar field.This is a 3-month contract position, with a possibility of renewal.The CSP Advocate will:Be involved in referral, screening, intake, assessment, and follow-up processes with children and their families.Ensure patients have a treatment plan with clear goals, objectives, timelines, and discharge plans.Identify and mitigate barriers for families to attend appointments and reach treatment plan goals.Perform regular check-ins to support families in achieving goals.Support research and evaluation activities in collaboration with Horizon Health Network research team.Support skill development and parenting strategies with caregivers.Maintain accurate and up to date patient records in the Electronical Medical Records (EMR) and Outcome Tracker (OT) databases.Conduct outreach activities with schools, government, community agencies, providers, and others.Support and assist with group programs (e.g. Creative Expressions, Cooking Up Justice).Promote the best interests of children in accordance with the United Nation's Convention of the Rights of the Child.Qualifications:A bachelor's degree in social work (BSW) is recommended.
