<p><strong>Title: </strong><strong>Nurse Practitioner</strong></p><p><strong>Employment Terms:</strong> Regular<strong>, </strong>Part -Time (0.5 FTE)</p><p><strong>Schedule: </strong>17.5 hours/week. Mondays: 1:30 pm to 5:00 pm, Wednesdays and Fridays: 9:00 am to 5:00 pm. Hours are subject to change based on program requirements and community needs.</p><p><strong>Salary Range:</strong> $53,928 ? 64,828 per year. It is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the Ministry of Health. HATP is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. <strong>This is an HATP-FHC position.</strong></p><p><br> <strong>Position Summary:</strong></p><p>The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services. As an independent professional the Nurse Practitioner will work to full scope of practice to contribute to clients? health providing assessment, prevention, education, rehabilitation and therapeutic support services consistent with the standards of care outlined by the College of Nurses. We are seeking a Nurse Practitioner who is committed to community based, client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.<strong> </strong></p><p><strong>Responsibilities include:</strong></p><p><strong>Direct Client Care:</strong></p><ul><li>Provide comprehensive primary health care including assessments, diagnosis, treatment plans, follow up, referral, education and referral within the scope of practice of a registered nurse in extended class</li><li>Maintain accurate client visits and findings on current record system</li><li>Coordinate client care within FHC and HATP and with other health care agencies and specialists</li><li>Determine the need for and orders and interpret screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds etc.</li><li>Counsel and provide information and follow up on abnormal results.</li><li>Prescribe designated drugs as specified by the College of Nurses of Ontario</li><li>Participate in the development and delivery of health services provided through HATP including: HATP primary care clinic (cradle to grave care), primary care for youth (ages 12 ? 25) at the Youth Wellness Hub and wrap around support with Thorncliffe Park community health service providers.</li><li>Follow clients in the community on scheduled home visits for clients unable to visit HATP locations</li><li>Participate in Quality Improvement Programs.</li></ul><p><strong>Health Promotion and Education:</strong></p><ul><li>Implement health teaching and promotion with clients as part of clinical assessm
