Collingwood Neighbourhood House (CNH) is a not-for-profit Society serving the Renfrew-Collingwood neighborhood of East Vancouver. CNH?s mandate is to promote the well-being of the Renfrew-Collingwood community by providing leadership and working collaboratively with individuals, families, agencies and other groups to develop and support inclusive, innovative, sustainable initiatives and services that respond to the community?s social, educational, economic, health, cultural and recreational needs.
RISE Community Health Centre (CHC) is the newest department of Collingwood Neighbourhood House. RISE is a community-based CHC that is part of the Primary Care Network strategy and funded by the Ministry of Health. RISE supports the health and well-being of Renfrew-Collingwood residents who are facing barriers to achieving health by offering community-based primary care and health promotion services. Populations of focus include Newcomers, LGBTQ2S+, Indigenous, Isolated Seniors, Youth, people experiencing Homelessness, those working in the Sex Trade, and those experiencing Mental Health and Substance Use issues. Services are delivered by a diverse interdisciplinary team, working in partnership with other Collingwood Neighbourhood House departments and partnering agencies.

*JOB OVERVIEW*
We are seeking a Community Health Worker to join the RISE Community Health Centre Team. The Community Health Worker acts a bridge between the community and the healthcare system, and is involved in doing intake for new clients, and supporting clients with Social Determinants of Health in one to one and group-based settings. They work closely with the social workers and the rest of the interdisciplinary team which includes family physicians and nurse practitioners, nurses, dietitians, physiotherapists, counsellors, medical office assistants, pharmacists and more. The Community Health Worker will be a staff of Collingwood Neighbourhood House, reporting to the Social Worker (Master?s of Social Work) and RISE CHC Coordinator.
*SKILLS & QUALIFICATIONS*
· Completion of a recognized Bachelor?s degree in community development or social services, or other related program· Two years recent, related experience providing community outreach, group facilitation, and social support to populations experiencing barriers to care, or an equivalent combination of education, training or experience
· An egalitarian philosophy and non-judgmental attitude (i.e. gender, culture, age, race, sexual orientation, language, ability and more)
* Working knowledge of community development values and principles
* Previous experience providing culturally safe, trauma-informed care to marginalized populations
* Ability to work both independently and as part of an interdisciplinary, culturally diverse team
* Experience applying strategies or practices to address social determinants of health and health inequity
* Extensive knowledge of community resources to support populations of interest
* Demonstrated ability to connect clients to community supports and resources
· Experience planning, organizing and facilitating community engagement sessions, workshops and events
· Ability to provide outreach in community settings and in people?s homes
· Ability to observe and recognize changes in clients and refer as appropriate
· Ability to deal effectively with conflict situations, manage challenging behavior, and use non-violent crisis intervention and behavioral management techniques
