ConnectWell Community Health is seeking a full-time Nurse Practitioner, ongoing, 35 hrs/week. This ongoing position provides a comprehensive benefit packages, HOOPP Pension and no weekend work.
*Position Summary:*
The Nurse Practitioner (1.0 FTE) works as member of an interdisciplinary team providing primary care to clients rostered to the NP at the CHC in Beachburg/Cobden/Eganville and includes primary care to youth age 12 to 25 at the Youth Wellness Hub in Pembroke.
Qualifications:
* RN (EC) license/NP Primary Health Care Certificate
* Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
* Three years experience in the nurse practitioner role in a primary care setting or community preferred
* Demonstrated ability to work effectively in an inter-disciplinary team
* Strong clinical skills
* Proven commitment to and knowledge of community-based health care, preferably in a rural community and with disadvantaged population groups
* Experience in the development and delivery of health education/promotion programs.
* Ability to work flexible hours, if required
* Valid driver?s license and use of a vehicle
* Attention to detail, ability to prioritize and time management skills
* Excellent interpersonal, oral and written communication
Reporting Relationship: The successful candidate will report to Director, Renfrew County.
